‘68 Rebel Sputters and dies

I’ve been maintaining my grandpas ‘68 Rebel for quite some time now, and I can’t figure out how to fix this issue. If I’m cruising anywhere from 35-50 (depending on the speed limit), then come to a stop due to a signal or stop sign, the car sputters and dies when I let go of the brake to accelerate.

Any tips or advice?
