Derby Della Lanterna is one of the most underrated derbies in all of Europe. The atmosphere at the Marassi is a bucket list match for me to attend.
That said, as a juventino it’s always Derby D’Italia
Honestly it’s any given Serie A team playing against Juventus. They always play like their lives depend on it

I’m going to keep the greatest derby out of my answer because I’m, well partisan.
So apart from Genova, I love watching the San Siro Derby. In the past they represented a majority of the national team!

For me, the derby of Italy is too loaded with tension, competitive anger, controversy before and after, often resulting in boring matches and few goals.
The real big match of the season for me is the Milan derby, which almost always result in exciting matches even when the two teams had rather poor rosters. Not to mention the choreography of the organized fans, always spectacular.
But I think few derbies in the world are more deeply felt by their respective fans than the Rome derby.
Full disclosure: I’m an inter supporter. When we play against Milan it’s rivalry… against Juventus, it’s personal.
